Registered Nurse (EHS)

- Conduct health risk assessments and physical evaluations for elderly clients, including cognitive function, mental health, nutritional status
- Accurately perform assessments using standardised tools and equipment, and document findings
- Provide personalised health education focusing on disease prevention and healthy aging
- Assist clients in managing chronic conditions and age-related health issues
- Organise health promotion activities and support clients in understanding assessment results and follow-up recommendations
- Act as case manager to process referrals to family doctors, specialists, and other health professionals
- Follow up with clients to monitor progress and ensure adherence to care plans, communicating assessment findings to the multidisciplinary team as needed
Entry Requirements:
- Holder of a valid Registered Nurse practicing certificate issued by the Nursing Council of Hong Kong
- Degree in Nursing or equivalent, with a minimum of two years of relevant nursing experience
- Experience in elderly health services and community health nursing, with good communication skills
- Basic understanding of common geriatric health issues and willingness to pursue qualifications in gerontology nursing
All candidates of the above vacancies should possess the following abilities :
- Strong command of both written and spoken English and Chinese
- Proficiency in PC applications such as Microsoft Word, Excel, and Teams
- Self-motivated, detail-oriented, responsible, and trustworthy, with strong interpersonal skills
- Effective communication and organizational abilities
- Complete any other duties assigned by supervisor(s)
