The challengesCommunicate with multi-disciplinary colleagues, patient and relative to work out the discharge plan. We should familiar to the prognosis and consequence of different medical condition related health condition progress, e.g. good rehab potential? bed-bounded? Walked with assistance? . As a MSW, we should give advice to the both short-term and long-term caring plan as well as explore the possible community support service for reference. On the other hand, we should familiar with the SWD service and allowance to strengthen the institutional care arrangement to facilitate the discharge planning.
